Beacon Health Options® On Track Outcomes: Overview

The Beacon Health Options® On Track outcomes program is designed to support network providers as they help clients stay “on track” in achieving their goals. This program is based on the completion of the brief Client Feedback Form (CFF) by the client during the course of receiving counseling services.

Using the Client Feedback Form (CFF)

The CFF is designed to help clinicians assess clinical risk and monitor changes in symptoms and functioning as individuals receive services.The completed CFF is reviewed by the clinician and faxed to a toll free number for analysis. Clinicians receive online reports based on these analyses. Clinicians may also find it helpful to review the CFF with the client during the counseling session.

On Track Outcomes is intended for use with clients receiving EAP counseling and commercially-insured psychotherapy services. Network clinicians may now use the program for all of their clients, including, if they choose, those who are not Beacon Health Options® members.

Note about EAP billing: On Track does not change the requirements for submitting the practitioner-completed Case Activity Form (CAF) for EAP billing. EAP providers should continue to submit the CAF to Beacon Health Options® as in the past.

Participating in On Track

Individual clinicians with access to the Beacon Health Options® ProviderConnect web portal can use the On Track tools by clicking on the program links after they have logged into ProviderConnect at The first time you use On Track you will be asked to confirm key information used by On Track before being connected to the On Track forms and tools. If you are not yet a ProviderConnect user, just click on the "Register" button on the login page to become a new user.

Note: Group practices and group practice administrators should send an email to for information about how to begin using On Track.

The ProviderConnect portal provides seemless connectivity to a secure personal web account where each clinician can view the CFF results for their clients. Detailed information about the On Track program, personalized copies of the CFF (adult, youth and child versions) with your NPI, Information for Clients, and other information can also be accessed on this web site.
