Which questionnaires should I use?

Verity Toolkit users presently have access to a set of questionnaires being employed by clinicians in Oregon and elsewhere. This initial prototype set of forms may be altered depending on feedback from clinicians and the needs of the Verity Outcomes Informed Care program. To access these form, go to: MyForms

Clinicians are free to user their clinical judgment when selecting which questionnaire to use.

Current choices include:
  • Adult Questionnaires
    • Well tested set of items
    • Spanish translation available
    • Targeted for outpatient treatment setting with patients of moderate to severe symptoms of depression, anxiety, social isolation and functioning in the workplace and/or other important tasks of daily living
  • Recovery Questionnaires
    • Well tested set of items used in substance abuse treatment program
    • Separate versions for individual or group treatment
  • Youth Questionnaires
    • Well test set of items targeted for adolescents
    • Balance of items assessing internalizing and externalizing symptoms
    • May be completed by the youth of parent/guardian
    • Spanish translations available
  • Child Questionnaires
    • Well test set of items targeted for children up to the age of 12
    • Balance of items assessing internalizing and externalizing symptoms
    • May be completed by the youth of parent/guardian
    • Spanish translations available
