How much will it cost me?

Tufts Health Plan will cover all of the cost for your access to the ACORN Toolkit and the use of ACORN questionnaires administered and submitted via any device with a browser and access to the internet, including smartphones and even inexpensive tablets with a wifi connection.

You may use the toolkit and forms with any and all of your patients, whether or not they are Tufts Health Plan members. Tufts Health Plan will cover the transaction costs for paper questionnaires completed by a Tufts Health Plan member and faxed to the ACORN secure fax server. For the first three months of participation, the health plan will also cover the transaction costs for non-Tufts Health Plan members. After the first three months, you will be responsible for transaction fees of $1 per form for questionnaires completed by non-Tufts Health Plan members. This charge is only for paper forms faxed in for data entry; there is no additional cost for forms submitted directly over the internet.
